Attention New Patients

If you are interested in being treated at O Physical Therapy, Inc., please be sure we have all the information necessary in order begin the scheduling process.  A referral along with a copy of your insurance card will be needed before the date of your first appointment.  We accept Blue Cross, Blue Shield and Medicare.  Unfortunately, we are not accepting United Healthcare, Medi-cal or Work Comp cases at this time.  For all other insurance policies please contact us for more information. We ask that new patients show up 15 minutes prior to the time of their first appointment to fill out our New Patient Packet  If you would like to download and complete the New Patient Packet prior to your first appointment we would greatly appreciate it.  Please  e-mail or mail your completed packet, allowing enough time to receive it prior to your first scheduled appointment. If you have any questions please give us a call and it would be our pleasure to address any questions or concerns you may have.
